C/O pain and swelling-left ankle/foot.
Diffuse irregularly defined hypointense areas on the T1W images which turn hyperintense on the GRASS and T2W images are seen around the ankle joint and midfoot. This would suggest hypertrophied synovium. The talus and calcaneum appear to be eroded.
Inflamed/hypertrophied synovium is seen to be hypointense on the T1W images and turns hyperintense on the T2W, STIR and GRASS images. Contrast(Gd-chelate) may be required to differentiate the inflamed synovium(enhances) from fluid. Chronic synovial proliferation(frond like) may reveal hypointense nodules or strands on the T2W images. Periosteal reaction/Bone erosions may be seen. Bone resorption, cartilage destruction, joint subluxation may be seen. Other changes suggestive of osteomyelitis may be seen. Inflamed/hypertrophied synovium may be seen with tuberculosis, PVNS(see hemosiderin) and JRA/RA.
